Back in the day I saw two such events.

One was self inficted when a young banger stuck his new Beretta in his waistband with finger on trigger.

We had a guy at my first duty station in Georgia that somehow emasculated himself with a 12 gauge. He didn't make it.

Ruled accidental, but of course all kinds of rumors flew.

this incident reminds me of the armed-robber who rammed his homemade bomb into his drawers a few years ago, where the blasting cap EXPLODED, removing the majority of his "goodies".
(the "main charge" failed to detonate.)

i suspect that he sings soprano these days in the prison choir!

When I was a teen, our pastor told me several stories about growing up in Texas. One involved a buddy of his who was an enthusiastic practictioner of the then trendy fast-draw craze. He apparently used a appendix type holster.

When the obvious OOPS! happened, he perforated his scrotum with a .22 slug, missing all important components. However, he had a couple of weeks in a hospital in a position very similar to that of a female having a pelvic exam. It seems every doctor, and many other medical types, would come in, read the chart, lift the sheet, laugh (or, if female, giggle, which was probably worse) and leave.

He never again participated in anything having to do with holsters & handguns.
